Extensive literature research together with intensive review of current practices in healthcare transition, is underway for the the purpose of outlining up-to-date efforts and evidence in the field. Doing so will not only help us assess need and identify barriers, but at the same time, keeping informed and exchange of information among collaborating agencies will promote communication and networking among partners. This increased sense of ownership for the shared interest, will help us produce quality resources for both providers and families. Outcomes shared in the community and among researchers can also help build a solid plate-form for ongoing and future efforts.
